SAFETY AND WELL-BEING RESOURCE GUIDE
The front of both the “BMS INTRODUCTION” and the “EARLY SEASON TEAM PLEDGE” are exactly the same. The front of the two versions provides information regarding how to locate and vet therapists/counselors and at the bottom there is provided safety and well-being messaging. In the center of the front page theBMSproject.org’s Top Three Athlete Tips are provided.
The “BMS INTRODUCTION” has information on the back that introduces theBMSproject.org; including a QR code that links to theBMSproject.org’s “By-Sport Directories.” The directories when accessed provide links to all of theBMSproject’s resources for the sport(s) selected.
The front of the “EARLY SEASON TEAM PLEDGE” is intended to be printed out by the coach and used as a meeting handout to promote team discussion (coaches, athletes and parents) regarding the provided Top Three Athlete Tips. The back of the handout is intended to be reviewed and signed by all meeting participants — coaches, athletes and parents — prior to or at the beginning of the season.

MID & POSTSEASON PLAY SAFE 2DAY SURVEY
Provided below is a link to a description of the mid and postseason mobile phone conducted survey that is still in development. The description of the survey can be downloaded and printed out and distributed prior to administration. Also, provided below is a link to register to receive future announcements regarding the introduction of the Play Safe 2Day survey. The front of the Survey Handout provides a description of the program. The back provides examples of questions, the multiple choice response format and an example of the response to one question from the aggregated, anonymous report that will be sent to managers and organization heads.

HELP A FRIEND 2DAY STICKER PROGRAM
This program comes in three versions: 1) A sheet of 14 vinyl all-weather stickers inserted into the Play Safe 2Day Program envelope to be distributed at sign-ins at team events, 2) vinyl all-weather Stickers-on-a-Roll to be distributed by organizers to individual athletes at sign-ins at camps and clinics. PLEASE NOTE: Both the Play Safe 2Day inserted stickers and the Stickers-on-a-Roll are printed on durable vinyl suitable for outdoor use.
