An announcement will be made to all BMS Project licensed affiliates when the Quick Pulse Metrics survey program is available for registration. Final proof-of-concept field tests are currently being conducted.
INSTRUCTIONS: Download the form and complete the fields in the form and email form to Registration@QuickPulseMetrics.com. Survey QR Code Transmittal Sheets will be emailed to each team’s manager (one unique QR code per survey). NOTE: An opt-out option will be provided prior to administering surveys to minors. A MESSAGE TO COACHES &
HEADS OF YOUTH SPORTS ORGANIZATIONS
QuickPulseMetrics survey program is offered to licensed youth sports organizations on a subscription fee basis. After a free trial period, the athletes’, parents’ and coaches’ mid and postseason surveys are provided as a package for an annual per team $35.00 subscription fee.
“Help-a-Friend 2Day” vinyl stickers are included as part of the survey program at no additional cost (one sheet per team). Blank Avery label paper sheets can be purchased locally at an office supply store 0or at Amazon here. In order to print the stickers at home a downloadable sticker template for Avery #5160 labels is available here. The “Help-a-Friend 2Day” stickers are designed to be distributed to athletes. The stickers’ template is preprinted with a QR code linking to theBMSproject.org website; as well as bearing a “988” 24/7/365 Intervention Lifeline printed reminder on the stickers.
DESCRIPTION
TEEN/ADULT SURVEYS (Typically for survey participants 13 years of age and older): Upon receipt, the team’s manager (head coach) and/or group leader should copy the mid and postseason QR transmittal sheets and hand them out to the team’s athletes/group leaders in an attended group environments, mid and postseason. In the presence of the manager (or survey administrator) the athletes will then scan the QR codes and complete the short multiple-choice surveys. Team results will be emailed to the head of the organization, the team manager and/or group leader. Organization MIS Summary Reports will be provided to the Head of the Organization when available. The Teen Surveys will be administered mid and post-season for the sport/activity being surveyed. The purpose of the surveys are to permit organizations to monitor their teams’/groups’ attitudes and emotional status; to contribute to the safety and well-being of participating athletes/survey participants and permit coaches/group leaders to improve their coaching/teaching/leadership skills. Assessment and responses to the survey feedback reports will be the sole responsibility of the participating team coaching staff/group leader and/or the organization. BMS does not conduct any evaluation or assign judgments and/or labels whatsoever. Individual survey participants’ identities will remain completely anonymous.
PARENTAL SURVEY(S) (Typically for parents of minor athletes 8 – 17 years of age): The Parental Survey may be administered mid and/or at the end of the season. The organization should set a target date for all teams/groups within the organization to administer the surveys. Again, mid and postseason QR transmittal sheets will be emailed to the head coach/group leader. The coach/group leader will then make and provide copies of the QR transmittal sheets to parents of each athlete/survey participant that can then be scanned permitting parent(s) to complete the surveys. Survey results will be distributed to the Organization Head and Team Managers via email. The method of administering the Parental Surveys will be left to the Organization to determine and prescribe. BMS does not assign any judgments whatsoever. Parental Survey participants will also remain completely anonymous.
COACHES SURVEY(S): The Coaches Survey may be administered mid and/or at the end of the season. The organization should set a target date for all teams within the organization to administer the surveys. Mid and postseason QR transmittal sheets will be emailed by the organization to each head coach and/or group leader. The coaches will then scan the QR code and take the surveys. Survey results will be distributed to the Organization Head via email — the organization can then share the survey results on whatever basis they deem most appropriate. BMS does not assign any judgments whatsoever.
