INSTRUCTIONS:<\/b>\u00a0 Quick Pulse Metrics survey registrants must be licensed users of the BMS Project’s programs.\u00a0 Click here<\/a> to apply if not already a licensee.\u00a0 The Quick Pulse Metrics survey program is an optional, for-fee program.\u00a0 The licensed, turnkey BMS Project programs are provided at no-cost to licensees.\u00a0 A portion of the proceeds of the Quick Pulse Metrics programs will be donated to assist to amortize costs associated with providing the no-cost\u00a0 BMS Project programs.\u00a0 Most surveys will involve teams and athletes.\u00a0 However, there may be instances that a registering organization does not have teams and\/or athletes, but wishes to conduct a survey.\u00a0<\/p>

Licensed registrants should complete the fields below, then click on “SUBMIT.”\u00a0 One sheet with fourteen peel-off vinyl stickers per team will be shipped to the Organization’s Home Office for further distribution to the teams’ managers\/survey administrators.\u00a0 A single-entity, survey registrant will have the QR Code Transmittal Sheet sent directly to the registrant.\u00a0 For organization registrants, the QR Transmittal Sheets will be emailed to each team’s manager\/survey administrator (one unique QR code for athlete mobile phone registration).\u00a0 In the instance of Parental and\/or Coaches’ Surveys, a QR Code and\/or unique link will be provided to access the survey.\u00a0 The provided link can then be transmitted to survey participants via email.\u00a0 If you have any questions, email survey@BMSHoldingGrp.com or call 213-924-0397 M-F, 9:00 am \u2013 5:00 pm Central Time Zone.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Description of Surveys<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

TEEN:\u00a0 <\/b>Upon receipt, Team managers will copy the QR Transmittal Sheet and hand out a copy to each athlete in a team meeting.\u00a0 In the presence of the manager (or survey administrator), the athletes will then scan the QR codes and complete the short multiple-choice surveys using their cell phones.\u00a0 Team results will be emailed to the head of the organization and the team manager.\u00a0 Aggregated organization MIS Summary Reports are being created and will be provided when available.\u00a0 Teen Surveys can be administered mid and postseason.\u00a0 The purpose of the Survey is to permit organizations to monitor their teams\u2019 attitudes and emotional status, to contribute to the safety and well-being of participating athletes, and permit coaches to improve their coaching skills.\u00a0 Assessment and responses to the survey feedback reports will solely be the responsibility of the participating team coaching staff and\/or the organization.\u00a0 BMS does not conduct any evaluation or assign judgments and\/or labels whatsoever.\u00a0 Survey participants will remain anonymous.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

PARENTAL:<\/b> The Parental Survey may be administered mid and\/or at the end of the season.\u00a0 The organization should set a target date for all teams within the organization to administer the surveys.\u00a0 No more than two weeks after the selected target start date, results will be distributed to the organization head and team managers via email.\u00a0 In most cases, the results should be able to be distributed within five working days of the survey being conducted.\u00a0 The method of administering the Parental Surveys will be left to the organization to determine and prescribe.\u00a0 BMS does not assign any judgments whatsoever. Parental Survey participants will also remain anonymous.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t
